This guide is for a normal sick day or a short run of them. It is not for surgery recovery, family leave, or a workers’ compensation claim. Those use different forms.
Step 1: Find the rule
Open the handbook, the PTO policy, or the last message from HR. Write down:
- How many days you can miss before a note is required
- Who receives the document
- The deadline after you return
If the policy is silent, a short email to your manager asking where to send a note is enough. Do that before you pay for a visit so you are not collecting a document nobody asked for.
Step 2: List the dates
Count the days you missed or will miss. Include only those dates. A note that covers a week when you missed one day creates a second conversation you do not need.
Step 3: Complete the visit
On GetMyDoctorNote you answer questions about those dates and why you were out. A licensed provider reviews the visit. When a note is appropriate, you get a signed PDF.
The visit price is $19.99. You do not go to a clinic for this step.
Step 4: Check the PDF
Confirm your name is spelled the way payroll has it, and that the dates match step 2. If they do not, contact support before you send the file.
Step 5: Send it once
Attach the PDF to the email or portal HR named in the policy. In the message, state the dates in one sentence and stop there. Do not add medical detail the note itself does not include.
Save the sent email. If someone asks again next week, you can forward the same file.
